Patient Care Coordinator 

We are seeking a dedicated and compassionate Patient Care Coordinator to join our healthcare team. The Patient Care Coordinator will be responsible for managing the administrative aspects of patient care, ensuring smooth communication between patients, practice team members, and providers. This role plays a vital part in delivering high-quality patient-centered care and ensuring a positive experience for all patients.

Responsibilities: 

  1. Patient Communication: Act as the primary point of contact for patients, responding to inquiries via phone, email, or in-person visits promptly and courteously. Provide information regarding appointments and general inquiries about the practice. 

  2. Appointment Scheduling: Coordinate and schedule patient appointments, ensuring efficient use of healthcare provider’s time and resources. Maintain accurate appointment calendars and follow-up with patients to confirm appointments and provide any necessary instructions

  3. Patient Intake: Collect and verify patient information, including personal details, insurance information, and consent forms. Ensure all necessary paperwork is completed accurately and in a timely manner.

  4. Medical Records Management: Maintain electronic medical records (EMR) and ensure that all patient information is kept confidential and up-to-date. Retrieve and file medical records as needed, adhering to HIPAA regulations and privacy guidelines.

  5. Patient Advocacy: Serve as an advocate for patients, addressing their concerns, and assisting them in navigating the healthcare system. Work closely with healthcare providers to ensure that patient needs and preferences are communicated effectively.

  6. Quality Improvement: Participate in quality improvement initiatives aimed at enhancing patient satisfaction, safety, and outcomes. Collect patient feedback and implement changes to improve the delivery of care and overall patient experience.

Qualifications: 

  1. Previous experience in healthcare administration, patient coordination, or customer service role preferred.

  2. Proficiency in medical terminology, EMR systems, and Google platform.

  3. Excellent communication, interpersonal, and organizational skills.

  4. Ability to multitask, prioritize responsibilities, and work effectively in a fast-paced environment.

  5. Accountability to open or close the facility 

  6. Strong attention to detail and accuracy in completing administrative tasks.

  7. Knowledge of HIPAA regulations and patient privacy laws.

  8. Commitment to providing compassionate and patient-centered care.

Salary and Schedule:

  1. Hourly rate of $15/hr with flexible hours Monday-Friday.

  1. 8-24 hrs/week. Willing to work around class or other work schedules.
